Malaria Control Efforts Across Africa: Progress, Setbacks, and What Comes Next


Malaria remains one of Africa's most persistent public health challenges — but it is also one of the clearest stories of what sustained, coordinated action can achieve. Understanding both the wins and the new threats is essential for anyone working in health, or simply anyone who wants to see the disease finally brought under control.

The Scale of the Problem
Africa continues to carry the overwhelming share of the world's malaria burden. The WHO African Region accounts for an estimated 94–95% of malaria cases and 95% of malaria deaths worldwide, with children under 5 making up about 75% of all deaths in the region. Globally, an estimated 282 million cases and 610,000 deaths occurred in 2024 — roughly 9 million more cases than the previous year.

The burden is also highly concentrated. Two-thirds of global cases and deaths are found in just 11 African "High Burden High Impact" countries, and four countries — Nigeria, the Democratic Republic of Congo, Niger, and Tanzania — account for more than half of all global malaria deaths.

The Progress We Shouldn't Overlook

It's easy to focus only on the grim numbers, but the long-term trend tells a more hopeful story. Since 2000, an estimated 2.3 billion malaria cases and 14 million deaths have been averted worldwide, including roughly 1 million lives saved in 2024 alone. Forty-seven countries and one territory are now officially certified malaria-free by WHO, proof that elimination is achievable with the right investment and political commitment.

Several tools have driven this progress:
● Insecticide-treated bed nets (ITNs) — mass distribution campaigns have made nets a household staple across malaria-endemic regions.
● Seasonal malaria chemoprevention (SMC) — this intervention scaled up to reach an average of 55–56 million children across 19 African countries in 2025, with Nigeria alone accounting for over half of all children receiving it.
● Malaria vaccines — since WHO approved the first malaria vaccines in 2021, dozens of countries have begun rolling them out, and 2025 saw a major milestone with the authorization of the first malaria medicine for young infants.
● Better case management — diagnosis and use of artemisinin-based combination therapies (ACTs) for children under 5 has improved, the most effective treatment available for the deadliest malaria parasite.

The New Threat: Drug Resistance:

The 2025 World Malaria Report's central warning is about resistance. Antimalarial drug resistance is growing and stands in the way of achieving malaria elimination, with partial resistance to artemisinin derivatives — the backbone of modern malaria treatment — now confirmed or suspected in at least 8 African countries. There are also early signs that the effectiveness of partner drugs used alongside artemisinin may be declining.

This matters because artemisinin-based therapies became the global standard precisely because older drugs like chloroquine and sulfadoxine-pyrimethamine had already failed due to resistance. If history repeats itself, the world could lose its most effective malaria treatment just as case numbers are climbing again.

Progress toward eliminating malaria deaths — a core target of the WHO's Global Technical Strategy for Malaria 2016–2030 — is currently far off track.

Gaps That Still Need Closing:

Pregnant women remain under-protected. In 2025, only 45% of eligible pregnant women and girls across 34 countries received the recommended three or more doses of preventive medicine — far below the 80% global target.
Fragile gains can reverse quickly. Even after achieving malaria-free certification, some countries have seen resurgence, underscoring that elimination requires ongoing vigilance, not a one-time victory.
New mosquito species are spreading. The invasive Anopheles stephensi mosquito — historically found in Asia — has been detected spreading across parts of Africa, raising fears it could establish malaria transmission in new urban areas that were previously low-risk.
Why This Matters for Health Communicators
For those of us working in infection prevention and public health communication, malaria control offers a powerful case study:
Simple tools save lives at scale — bed nets and seasonal prevention campaigns show that low-cost, well-targeted interventions can avert millions of deaths.
Complacency is dangerous — resurgence in "malaria-free" countries proves that surveillance and community engagement must continue even after apparent success.
Community-level education is critical — ensuring pregnant women, caregivers, and community health workers understand prevention options directly affects whether coverage targets are met.
Resistance is everyone's problem — the same principles driving antimicrobial resistance across other diseases apply here: misuse and inconsistent treatment access accelerate the loss of effective drugs.

The Bottom Line

Africa's fight against malaria is a story of real, measurable progress being tested by real, growing threats. The tools that got the continent this far — bed nets, seasonal chemoprevention, vaccines, and improved case management — are still working. But drug resistance, funding gaps, and incomplete coverage for the most vulnerable groups mean the next chapter of this fight will require even greater investment, innovation, and community-level vigilance to protect the gains already made.
